Before we can respond to your enquiry about migration, we need detailed information about you and your circumstances (please attach a separate sheet of paper if necessary). All information given will be held in the strictest confidence. Please complete this questionnaire and then forward your answers to us together with your assessment fee of US$95 (if your application is successful this initial fee will be credited back to you). We will promptly arrange for the assessment of your case and contact you as soon as possible of your migration opportunities. We look forward to being of service to you.

2. Please write the names, male/female, dates of birth and relationship to you of all family members to migrate to Australia.

3. Are you married / divorced / separated / engaged / retired / over 55 years of age?

4. Please write your full address, telephone and fax numbers or email address.

5. Where do you plan to live in Australia and what do you intend to do for employment?

6. What is the approximate value of assets (finances) you will bring to Australia?

7. How would you rate your English language - Excellent / Very Good /Good / Poor? Have you taken any English language tests? If yes, write the name and date of the test and score achieved.

8. Please provide full details of your education and qualifications, including name of institution, dates of study, grades achieved, apprenticeships completed, special training undertaken, licences and registrations or memberships you hold. (Attach a separate sheet if necessary.)

9. Please write full details of your work history, including job title, dates of employment and work duties. (Attach a separate sheet if necessary.)

10. Please provide similar information about your wife's/husband's education, qualifications and work history. (This is important because in some cases it is better to make the other partner the primary applicant.)

11. Do you have any relative or potential employer in Australia who might be willing to sponsor you? Please give details.

12. Please provide details about any other dependants (not those mentioned in Q.2), whether migrating with you or not.

13. Have you or any of your dependent family ever had any serious disease, condition or disability? Please provide details.

14. Have you or any of your dependent family ever been charged or convicted of a crime or offence in any country? Please give details.

15. Have you or any of your dependent family previously been to Australia? If yes, please provide details.

Migration agents are independent of the Department of Immigration and must be registered by the Migration Agents Registration Authority (MARA) which sets quality standards and controls. Many agents are also members of the Migration Institute of Australia (MIA). A migration agent can provide fast service and can be useful in suggesting available options. An agent will prepare your application correctly so that it meets Australian migration regulations as these regulations may change frequently. There are over 100 different classes of visa and SMCA will inform you which is appropriate for your case. There are usually set fees for different types of visas and services.
